Responsibilities:

The successful candidates must be able to provide footcare service to clients both in home and clinics.

Key Responsibilities

• To establish, extract, evaluate and document the needs of the business with regards to custom application development
• To serve meals to residents and guests in a professional and courteous manner;
• To assist in the set up and preparation for service of various items such as beverages, salads, desserts, etc.
• To set and clear dining tables and deliver tray service as required;
• To assist with cleaning of dishware, cutlery at the conclusion of each seating as required
• To assist with maintaining the cleanliness of kitchen and dining area as required
• To assist with catering, and actively participate in special occasions and theme events
• To assist in the orientation and training of new dietary employees
• To follow and comply with health and safety legislation and departmental safe job procedures

Responsibilities:

• To assist in the collection and sorting of soiled personal clothing and linens and pre- treat stains as required;
• To load and unload washers and dryers ensuring that proper settings are used and that chemical supplies are at an appropriate level;
• To inspect soiled linens or personal laundry and complete any required repairs, e.g. rips, tears, buttons;
• To fold , segregate and stack linens and residents' clothing and ironing as required;

Job Summary:

Registered Nurses will provide professional nursing consultation services on a wide variety of nursing issues related to the care of older adult clients. You will promote programs to meet community needs, and provide exceptional nursing care to clients and caregivers in the community by applying VON Branch philosophy, policies and procedures.

The successful candidate must be available to work early mornings evenings and weekends as well as rotations, 1 in 3 weekends as well as one on-call assignment per month.

RESPONSIBILITIES

• Ensure resident care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents. Assure observance of required techniques in nursing care.
• Assist in assessing residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, notes changes in condition and assess need for referral to doctors; processes and implement physicians’ orders.
• Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refers problems to the Director of Care.
• Assist in preparing the Facility for RAI MDS.
• Take charge of the Facility in the absence of the Director of Care.
• Staff replacement in the absence of the care clerk.

Reporting to the Director of Health & Wellness, the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

Key Responsibilities

• To work as a member of the inter-professional team as described by Revera’s Professional Practice Model applying the person centered approach to care;
• To complete and ensure resident’s clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required;
• To implement and evaluate the effectiveness of the care/service plans and make changes as required;
• To assist with scheduling of resident’s care conferences and participate in conferences where applicable;
• To work closely with the Director of Health & Wellness to facilitate resident’s move in, move out and transfers;
• To provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Revera Resident Care standards and policies and procedures;
• To process the orders and safe keep drugs and narcotics and administer in compliance with all regulatory requirements, scope of practice and Revera policies and procedures;
• To facilitate and provide resident/family education;
• To plan, coordinate, manage & evaluate the resident’s care plan working with the resident/substitute decision maker, physicians, nursing & interdisciplinary team and contracted service providers;
• To coordinate nursing team’s resident care assignments to ensure efficient and timely delivery of services to residents;
• To respond to and manage significant changes in resident’s condition and or medical emergencies; provide emergency first aid;
• To teach, delegate to, and direct unregulated care providers;
• To coordinate emergency response in the absence of Director of Health & Wellness/ Executive Director;
• To respond to resident/family concerns and ensure appropriate action taken and or concern communicated to Director of Health & Wellness/ Executive Director and other departments;
• To follow and assist the Director of Health & Wellness in the coordination, implementation and monitoring of Infection Prevention and Control , Health and Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department;

Mission:

The Director – Memory Care Program is responsible for implementation and oversight of the Memory Care Program and serves as the leader and expert in Dementia/Alzheimer’s Memory Care for the organization.

The Director – Memory Care Program will be responsible for overseeing and standardizing operational processes to include financial cost efficiencies and provision of quality care and quality of life services to residents in the program according to the legislative requirements.

Key Responsibilities:

• Facilitate the execution of the implementation of standardized Revera Memory Care Program practices;
• In collaboration with the Clinical leadership team, lead and direct all services and programming within the Memory Care Program ensuring exceptional quality of life care and services;
• Work in collaboration with divisional leads to develop, plan and lead the execution of a broad range of quality of life activity programming to engage staff, family and residents;
• Ensure implementation of the comprehensive Memory Care training;
• Recommend and evaluate staffing performance according to the memory care program requirements, meeting care and service standards and budget allocations;
• Works as a team member to enhance or improve existing materials or develop new materials for use in dementia care programs;
• Communicates any changes to provincial regulations within both divisions;
• Audit patterns of behavioral incidents with staff and residents to determine trends and areas for improvement;
• Serve as the organizational leader and expert in dementia/memory care keeping abreast of the most current research and innovative treatments;

Reporting to the Director of Care, the incumbent will be responsible for serving the care needs of our Residents, within the scope of practice. This includes:

• Maintaining resident care plans and required documentation in accordance with Company policies and procedures and legislative requirements.
• Manages medications as prescribed and observes/reports changes in condition; monitors the safekeeping of drugs and narcotics
• Assists in maintaining a clean, safe environment for residents and staff
• Liaises with family members and acts as a resource for questions, concerns regarding practices or related health care issues.
• Perform other duties as assigned.
