A day in the life of a Dining Room Supervisor:

You will be responsible for the overall smooth operation of the Dining Room, in the absence of the Dining Room Manager. The Supervisor will demonstrate excellent judgment, self motivation and reliability as well as knowledge of dining elements involved in the understanding of the operation. The Supervisor will fulfill this role to ensure the procedures outlined are carried out and the principles of “5 Star” service is extended to all residents and guests.

A day in the life of a Director of Culinary Services:

You will be responsible for the smooth operation of all aspects of dining services within the Community to provide an exceptional dining experience to the Residents. You will foster outstanding teamwork and motivation and provide strong leadership to your team.

You will be responsible for the nutritional and dietary needs that ensure the well-being of our residents. Responsibilities:

• Receive and direct all incoming calls via telephone and voice mail in a timely and professional manner
• Responsible for greeting all visitors to the Home Help office, ascertaining the nature of their business, and directing them appropriately
• Responsible to maintain and excellent working knowledge of telephone/voice mail systems, equipment and procedures
• Coordinate and maintains accurate centralized phone, fax, pager and email lists of site employees
• Prepare mail or courier all items for scheduled pick-up

Responsibilities:

• Leads weekly classes under the direction of a regulated care provider
• Works toward fulfillment of client goals as outlined in their assessment
• Participates in on-going review of service delivery process and evaluation
• Attends any educational in-services that are deemed appropriate by the SMART Coordinator

• Reports any situations that are deemed uncomfortable to their supervisor/manager

Job Summary: 

The Physiotherapist (PT) is responsible to provide therapy services to individuals in the community within the framework of VON policies, procedures, standards, and quality and risk management. The PT works collaboratively with a Therapy Assistant to provide therapy services using an innovative care delivery model and a cutting edge electronic monitoring system.

Demonstrates knowledge, skills and attitudes as outlined in the Essential Competency Profile for Physiotherapists that will provide a safe and evidence informed practice.

Responsibilities:

• Consults with the patient to obtain information about their health, associated history, previous health interventions and outcomes
• Collects assessment information relevant to client's needs and physiotherapy practice
• Analyzes assessment findings to establish a physiotherapy diagnosis and prognosis
• Develops and recommends a plan of care with the patient/family and health care team.
• Directs the physiotherapist assistant to implement the patient's care plan
• Ongoing, in collaboration with the physiotherapist assistant monitors patient outcomes and progression towards therapy goals
• Modifies the patient's care plan as required based on patient outcomes
• Documents patient outcomes and current care plan in the electronic patient chart for the interdisciplinary team
• Communicates with interdisciplinary team as required regarding patient outcomes and status changes (for example most responsible physician).

Job Summary:

The Occupational/Physiotherapist assistant (OTA/PTA) under the direction of a Directing Therapist, provides therapy services to individuals in the home within the framework of VON policies, procedures, standards and quality & risk management. Collaborates with members of the health care team to meet the needs of individuals to attain an optimal level of functional and physical independence and well-being in their home environment.

Responsibilities:

• Performs the activities of daily living and associated exercises as directed by the therapist. Activities/responsibilities may include:
• Verifying client information by interviewing client under the direction of a therapist
• Reviewing medical history
• Confirm purpose of care plan with the Directing Occupational Therapist /Physiotherapist
• Collaborates with patients to improve their quality of life by helping them compensate for limitations, such as, teaching patients to dress themselves independently, bathe themselves, to prepare meals and/or to access community resources to increase their independence
• The OTA/PTA will follow a care plan (treatment plan) completed by the Directing Therapist. Focus on a holistic approach that assists the patient to reach their goals. For example
• Activities of daily living
• Use of equipment (patient assisted devices and modify the home environment to ensure patient safety and independence
• Safe transfers in the home environment
• Exercise program (optimize functional and physical abilities to improve strength and endurance)
• Monitor and report patient outcomes to the Directing Therapist
• Under the Direction of the Therapist will perform standardized functional and physical assessments (i.e. Berg balance scale)

The Client Service Specialist processes referrals by admitting the client on the system, establishing ongoing service and setting up the client file and communication lines with the client. The Client Service Specialist accurately and precisely schedules and allocates client service staff to meet the needs of Revera’s clients/patients while demonstrating excellent customer service and front line communication skills.

Hours are primarily evenings and weekends, but training will take place during the daytime

Key Responsibilities
 

• Intake of all referrals/service changes from multiple sources.
• Fill and schedule shifts through the availability and allocation of qualified staff with skills to match client needs.
• Enter service codes and pay rates with absolute precision.
• Communicate changes in staff hours/days to clients and referral sources and advise staff of changes in service schedules.
• Maintain and update GoldCare system client and employee files, including discharges.
• Manage client and employee service changes, availability and overtime.
• Track all client and staff concerns, complaints and compliments electronically when supervision staff is not on site.
• Provide exceptional customer service to current clients and prospective clients by responding to issues and resolve scheduling problems.
• Run weekly reports as stated in each branch and take appropriate action based on the data.
• Confirmation of employee schedules and travel time.
• Monitor telephony records and reconcile errors.
• Investigate billing rejections and follow up according to branch process.
• Work in compliance with the provision of the Occupational Health and Safety Act.
